REGISTRATION INSTRUCTIONS 

Dear GLAHA-Lansing Capitals Midget AAA Player:

 

Congratulations on being selected to play on our Midget AAA team.  On behalf of our organization we welcome you and thank you for joining us.  We are looking forward to an exciting and competitive season.

 

Please complete each of the steps listed below.  You can mail your completed forms and your deposit to:

 

Lansing Capitals Midget AAA

721 N. Capitol Avenue,

Lansing, Michigan 48906.

 

1. FORMS:

 

1) REGISTER ONLINE with Greater Lansing Amateur Hockey Association (GLAHA.) You can do this on the website at www.glaha.org . 

 

You can select your payment options at this site (or see options below), however, please note that the Deposit for MIDGET AAA should be paid directly to Lansing Capitals Midget AAA Hockey and it will be credited towards your total bill.

 

2)  REGISTER ONLINE with USA Hockey for the 2007-2008 season.

www.usahockey.com    Print a copy of the confirmation to send in with your

forms.  

 

3)  Fill out the following attached forms.  These forms can also be found at the Michigan Amateur Hockey Association website at www.maha.org .  These

include: 

 

          USA Hockey Consent to Treat Form

          USA Hockey Participant Code of Conduct

          STAR Parent's) Code of Conduct

 

4) A copy of your BIRTH CERTIFICATE.

 

5) Complete the attached PLAYER REGISTRATION.

 

2. PAYMENT SCHEDULE AND OPTIONS

         

1)  A $500 deposit is due now and an additional $500 by August 1, 2007. 

 Payable to: Lansing Capitals Midget AAA Hockey (LCMAAA).

 

2)  The remaining payments may be made by the following methods:

 

a) Checks payable to LCMAAA, or

B) Online Credit Card payment through GLAHA

 

Your fees include:  Ice fees, off-ice training, travel by charter bus, hotels, tournaments fees, jerseys and bags.

 

The fees do not include:  Warm up apparel, food or housing.   Fundraising

activities will be undertaken to cover these and any other additional costs.

Players are responsible for their own personal equipment and gear.

 

3. SUMMER MEETING

 

A parent/player meeting will be scheduled for Tuesday June 26th at 6:30 PM.

Items covered at the meeting will include:

 

 

I am looking forward to the upcoming season with you as a member of our organization.  You can expect to work hard, improve your hockey game to the next level, and have fun.  Be sure to take your summer off-ice program seriously so that you are ready for a high level of play when the season starts.
